Adoption Documents

Take a few minutes to review some helpful documents!

All documents need to be filled out and sent to our email johnsons_family_shepherds12@yahoo.com. You may scan the documents using a scanner or items using a smart device such as apps to get the documents sent to us like cam scanner or docusign (i am sure there are many others).

STEP ONE TO BEGIN ADOPTON: If you would like to fill out an application to be concidered to adopt a puppy click here.

STEP TWO FOR ADOPTION: If your application has been acepted and you were contacted by someone from Johnsons Family Shepherds and are ready to make your deposit click here.

STEP THREE FOR ADOPTION: Our puppies are very important to us. Agreeing to the puppy contract before purchasing a puppy is extremely important and protects the health and well being of our puppies. Please read the puppy contract carefully. Reading this ahead of time is beneficial. Once the puppy is paid for in full and will be picked up the contract will be completed with us. RIGHT TO SURRENDER: In some occasions a puppy/dog may need a new home. These situations happen and we understand that. Sometimes there are reason's beyond the owner's ability to fix. Johnson's Family Shepherds offers an alternative to dropping your dog/puppy off to a pound or a new unscreened owner. Please read our contract to understand. Click here for the surrender contract.

RIGHT TO SURRENDER AGREEMENT FOLLOW-UP: When a person has previousely surrendered a dog and they want to complete the agreement within the contract they must fill out this contract. Click Here
